Complete and submit the following items to the Admissions Office:

1. Application
A. All questions on the application must be completed. If a question does not apply to you, write N/A (Not Applicable) in the space provided.
B. Please include a recent photo of yourself (head and shoulders only). School photos are ideal.

2. Pastor’s Recommendation – If you pastor is your parent or spouse, ask another member of the church’s pastoral staff to complete this form.

3. Personal Recommendation – This form should be completed by a high school or college teacher, an employer or a friend. This person should not be a relative.

4. Health History – A recent physical examination is required.

5. Transcripts – Submit official transcripts from your high school and any college-level courses taken. Transcripts should be sent directly from your school to the CFNI Admissions Office.

6. Application Fee – A non-refundable $50 application fee is required with each application.
